The practice

Most medicine starts with a prescription. We start with the work.

Conventional primary care is built for volume. Seven minutes, a refill, a referral. There is no time to coach a squat, look at sleep, or take a medication away.

Knotmedicine keeps a panel small enough that your physician can do those things. Membership is the structure. Lifestyle is the method. Medication is what remains when the method is not enough.

Knotmedicine ties typical modern medical care — the exam, the diagnosis, the medicines that earn their place — to the way of living that actually makes a person healthy. Strength work, cardiovascular training, sleep, and food are part of the medicine, not a pamphlet you get after the visit.

Virtual visits

When the visit does not need a room, it does not get one. Video when it is the better tool.

Training is not a wellness extra here. It is the first-line treatment. Every member gets an individualized plan for strength and cardiovascular work, reviewed the way other practices review labs.

    Lift

    Strength work as a clinical plan. Loads, frequency, and progression written for your body — not a generic printout.

    Condition

    Cardiovascular training with a target. Zone work, intervals, or long easy miles — whatever the evidence and the calendar support.

    Recover

    Sleep and stress treated as vital signs. If they are broken, the rest of the plan will not hold.

    Eat

    Practical food you can live with. No protocol theater. Protein, produce, and the habits that actually stick.

If a medication can come off, we try. If it must stay, you will know why.

The physician

Michael Kellner, MD

Board-certified, emergency medicine

Dr. Kellner trained at Columbia University’s Vagelos College of Physicians and Surgeons and completed emergency medicine residency at the University of Connecticut. He has practiced in the Greater Hartford hospitals that take the region’s sickest patients.

That background is the point of a home-visit practice: the judgment used when minutes matter, applied with hours instead. Acute assessment in the living room. A training plan in the follow-up. A smaller medication list over time.

Membership

A few dozen people. Not a few thousand.

The panel is the constraint that makes the rest possible: same-day or next-day visits, unhurried exams, a physician who answers, a plan that gets revised when you miss a week of training.

Membership is annual and discussed in a private conversation. It is not billed to insurance. Labs, imaging, and specialists remain between you and those providers.

We take the next member when there is room to take them well.

Is this health insurance?+

No. Knotmedicine is a membership practice. Keep your insurance for hospitals, specialists, imaging, and labs. The membership is for the relationship with your physician — access, time, and a plan.

Do you still prescribe medication?+

When it is the right tool, yes. The aim is fewer medications, used with intent — not none at any cost. If a drug can come off, we try. If it must stay, you will know why.

House calls or virtual?+

Both. Onboarding and exams happen in person, in your home. Follow-up, training review, and many acute questions happen by video or message. Same-day or next-day house calls when you are unwell and it is clinically appropriate.

How many patients?+

A few dozen on the panel, and five new members in any given month — not more. That ceiling is the product. It is how your physician can answer, show up, and stay with the plan.

What if I have an emergency?+

Call 911. Knotmedicine is not an emergency department. After-hours, members have a defined way to reach the practice; life-threatening symptoms still go to emergency care. Call us as you are going — we can walk you through it, and if it is appropriate we may detour you for assessment, testing, or medication.

Who is this for?+

Adults in Greater Hartford who want a small practice, direct access, and a training-first approach to health. People who would rather lift, run, sleep, and eat well than collect prescriptions.
